Today's locker Talk and Shots topic is penis size. Questions slash concerns answered Big ones, little ones, fat ones, skinny ones we're going to answer and solve them all. So at the beginning of this year I published an episode called Sex with Small Penises the truth. I did a little round table discussion with four other women about our experiences having sex with small penises. As you can imagine, it was very popular and I'm guessing, if you're listening, you may have come to my podcast because of that Now.

Speaker 1:

As a result, I got lots and lots of questions, comments from men, and I have brought urologists on to help discuss penis enlargement, other penis problems. But apparently I did not do a good enough job, because I have gotten more and more questions and I have come to learn that more than 50% of men wish their manhood penis cock was bigger. I also received some feedback that many men who listened to that episode wished that my expert had been a man. I brought on two experts. They were women and men were like we'd like to hear from a man who actually has a manhood. Well, my expert today fits all of the criteria you've been asking for. Not only is he highly qualified to answer questions, he's also well a man and has a penis. So I'm excited to introduce him to you today.

Speaker 1:

His name is Dr Judson Brandeis. He is an award-winning urologist and sexual medicine expert specializing in the emerging field of sexual health. A graduate of Brown University and Vanderbilt University School of Medicine, he completed his urologic surgery residency at UCLA and a postdoc fellowship at Harvard. With over 25 years as a board certified urologist, he has performed thousands of surgeries and pioneered surgical robotics Impressive. In February of 2019, he opened BrandeisMD, a national leader in sexual medicine innovations for men, focusing on clinical research to enhance well-being and provide tools for men to live their best lives. Welcome, doctor. I would love for you to take a moment and just tell my listeners a little bit more about yourself.

Speaker 2:

Well, I think you wrapped it up pretty well, but I'm highly educated and almost entirely focused now on sexual medicine, hormonal medicine and physical optimization for men, and I invented what's called the P-Long protocol, which is the first natural, safe and effective way to increase the length, girth and function of a healthy guy's penis.

Speaker 1:

And so he just gave away the why you should stay until the end, because this is a question I get. Oh yeah, you're going to. You guys, you're going to want to stay to the end because we are going to talk about that solution, along with other solutions, but that solution in particular, because one of the recurring questions I have from men who are concerned about size is really what is the best solution? And there's a lot, of, a lot of, you know, nervousness and concern about it, and we are going to talk about that after we answer your questions. You sent these to me, so stay to the end, for you know the big bang, if you will. All right, I'm ready. Are you ready to talk about I'm ready Penis enlargement Cheers. I'm just going to start with. I'm going to add in some of my own questions when do you think, as someone who is highly educated, trained in the area of penis size, where do you think this concern, obsession with penis size, comes from?

Speaker 2:

Well, I mean, the penis is basically a proxy for manhood, and so the bigger the penis, the bigger the man. Same thing with muscles, same thing with cars, same thing with houses. For a lot of things, the bigger is, the better.

Speaker 1:

So you feel like it's mostly rooted in a societal expectation, because in my podcast and when I've talked with women, we don't tend to be as obsessed with the size of a penis. We're looking for satisfaction, which can come from lots of other things. But when I did that podcast, what I received back from men was a lot of women are liars. That's not what they really think and feel, and I'm like. So I was just curious as a man what an expert how you viewed that.

Speaker 2:

Yeah, I mean, it's not the size of the nail, it's the hammer that drives it. You know you can have a small penis and pleasure a woman. You can have a big penis and not pleasure a woman. I think it and pleasuring a woman, it's very, very complicated. It's not just the size of the penis. I think it's a man's way of being very simplistic, but obviously you know, and your listeners know, that it's a lot more complicated than that.

Speaker 1:

Yes, thank you. Thank you for clarifying. Another question I got was how to properly measure the length and girth of a penis.

Speaker 2:

Well, you know, first of all. First of all, half the penis is on the inside and half the penis is on the outside. So if you really want to show off, you can basically double the size of the outside of the penis and say that you have that much and not be wrong. There are different ways that people measure the penis. Some people just measure the penis from the skin and some people do what's called a bone press, where they push the ruler against the bone and measure the penis, and really you should measure the erect penis, because flaccid penis obviously changes size a lot, and so as long as you're doing it consistently the same way, I think that's all that really matters. But if you're a skinny guy and you do a bone press, you're only gonna push in the skin and the subcutaneous fat maybe a half an inch or an inch, whereas if you're heavy you can have a pannus that's two, three, four inches thick, and so that may significantly reduce your functional penis size.

Speaker 1:

All right, there you go, guys. Now you know there are options, and whatever you want to tell us, go ahead and tell us, because, as I've told you, we don't really care Exactly.

Speaker 2:

And you know, the interesting thing is that the folks that signed up for the P-Long study, they weren't all guys with small penises. There were guys with small penises, guys with normal size penises and guys with large penises.

Speaker 1:

Which brings me to my next question. I mean, I don't know that there's a scientific answer to this. What is the ideal penis length girth for men? When they come in, what are they like? Hey, I'd like a solid. What?

Speaker 2:

seven, eight inch, or what are they dreaming of? That's a good question and honestly I don't have a good answer for that. But we just added something to the P-Long website which should be really interesting. It's a penis size calculator, and so the P-Long protocol is a six month protocol where you can add about 15% to the length of the penis and about 10% to the girth of the penis, and so guys will put in the length of their penis and then six months later, this is what you can expect.

Speaker 2:

But the other thing that we're going to put in is also a sort of like a dream size calculator. So say you have a five inch penis and you want an eight inch penis, how many months of stretching and traction and PRP and taking the Affirm Nitric Oxide Boosting Supplement will you have to go through in order to get from five inches to eight inches? Because the growth is about two and a half percent per month for length and one and a half percent for girth, and so it'll be interesting to see what number people put in for their ideal penis size.

Speaker 1:

So that's almost going to double as a study for you. You'll be able to see, kind of what people are shooting.

Speaker 2:

Yeah, you know I'll. In a couple of years I'll come back and I'll let you know what that number is.

Speaker 1:

I would love to know that. I feel like in the comments, one thing that happened in the comments section of that episode was that every man who ever listened decided to share their penis size with me, Like I mean, they would just drop their penis size, and I feel like what I kept hearing was like eight inches was their dream. I don't know where eight inches is the number they came up with. I was like that feels like large to me, but I'm a small woman and I, as, as I've talked with a lot of other women, we mostly just want men who fit, like you know, a perfect fit, which could be anything.

Speaker 2:

Yeah, I mean once you get to the cervix. There's not a whole lot of a point to go beyond that.

Speaker 1:

I agree. I agree. Um. Another question that came up regularly was questions about growers versus showers right, that's what they call them. I'm like what causes some men to shrink, to appear to be so small and then actually end up being giant? Can you share a little about that?

Speaker 2:

And it's mostly based on blood flow. And so if you have a decent amount of blood flowing through the penis in the flaccid state, it's going to appear larger. And if you have very little blood flow flowing in the penis in the flaccid state, it's going to appear a lot smaller. And you know, part of the job of the vascular system is to restrict blood flow to the penis unless you're procreating, right? You know there are basically two functions of the penis One is for urination, the other is for procreation, and so as long as you have a penis size adequate for urination and procreation, you're fine.

Speaker 2:

And when you're not procreating, there's no reason for blood to be flowing through the penis, right? You want blood to be flowing to other parts of your body, like your brain or your muscles or your liver or your kidneys, flowing to other parts of your body, like your brain or your muscles or your liver or your kidneys. But people have different amounts of blood flow flowing into the penis and the flaccid state, and sometimes we can take advantage of that. So, for example, I have a lot of patients that are gay, that are interested in flaccid penis size because they walk around the locker room or wherever and they're looking to attract other guys, and so we can actually inject botulism, neurotoxin or Botox into the penis and that reduces the tone of the vascular smooth muscle and allows more blood flow into the penis in the flaccid state, and so you actually get a larger, flaccid penis by injecting Botox.

Speaker 1:

Wow, okay, so that's something I have never heard of. So, for personal aesthetic reasons, some men want to hang long when they're limp. My question is this so it's not a bad sign of health if they don't show when they're limp?

Speaker 2:

No, I mean there's no, you know, from a, from a evolutionary perspective. There's no reason to show much of your penis when you're not in procreation.

Speaker 1:

All right, well, there you have it, guys. There, there you have it.

Speaker 2:

If you look at animals. I mean, I don't look at animals' penises all that much, but I'd pretty much be willing to bet you that most animals don't show much of their penis, if at all.

Speaker 1:

It seems like it's going to be safer if it's tucked inside.

Speaker 2:

And safe if it's tucked inside. Especially if you're out in the wilderness running around being chased by animals, chasing other animals, climbing trees, doing all those kinds of things. You want it tucked away safe.

Speaker 1:

Yes, I would think so. I mean, that's my experience is I feel much safer because I don't have anything hanging out. I feel like it would be very uncomfortable watching men, I think how uncomfortable, but Well, same thing with the clitoris, right?

Speaker 2:

I mean, you have the clitoral hood. Why do you have a clitoral hood? Because it needs to be protected.

Speaker 1:

It does, it does. Girth came up a lot. A lot of men just asked what about girth? What about girth? What are your thoughts? Opinions on length versus girth, the importance, the effect.

Speaker 2:

Yeah, I mean, I think men care more about length because it's something that's much easier to compare, and I feel like women care a lot more about girth because that's, you know, like I said, once you get to the cervix there's not really anywhere else to go, not really anywhere else to go, and uh, but in terms of girth, you can stretch the vagina, uh, and maybe even access the clitoris by increasing girth, uh. And so, from what I understand, um, girth is much more important than that to women.

Speaker 1:

I would say that's definitely true. I hear most women complain about super skinny penises, like just jabbing their cervix but not bringing them pleasure.

Speaker 2:

So you know, the guys that do the jelking or just use a traction device get what we call in our scientific community pencil penis.

Speaker 1:

So one of my other questions was I had never heard of jelking, Can you explain what it is? And I think the questions I got were is it safe, Is it effective?

Speaker 2:

Yeah, and so, honestly, I don't know all that much about jelking, but I think it's like a technique to pull the penis and create like a traction on the penis. Now I think what a lot of it is is stretching the suspensory ligament, which is the ligament that attaches the penis. Now, I think what a lot of it is is stretching the suspensory ligament, which is the ligament that attaches the penis to the underside of the pubic bone, and so you're probably not actually increasing the size of the penis, you're just increasing the amount of penis that's external to the body.

Speaker 1:

It doesn't sound like a good idea to stretch ligaments.

Speaker 2:

I mean, I don't know about ligaments on penises, but yeah, you know, I've never actually seen a validated scientific study that shows that consistent jelking increases the length of the penis in any significant way.

Speaker 1:

Circumcision. Can we talk about that for a second? Does it affect penile size or sexual pleasure?

Speaker 2:

Well, and I do a ton of circumcisions, and so I do them for all sorts of reasons. Some people get infections, some people get bleeding, some people aren't able to access the head of the penis. So if you can't access the head of the penis, right then you're. The likelihood of you having sexual pleasure is pretty small, and so clearly a circumcision is warranted. In that respect, the knock on circumcision really has been a decrease in sensation. But to be honest, I've done hundreds and hundreds of circumcisions. I don't think I've ever had anyone complain that their penile sensitivity was a lot less, and so it's a safe procedure. It's a common procedure, and you can certainly live a long happy life and have sexual pleasure with a foreskin. But the flip side is, you can have a long happy life with a circumcision, whether you get it when you're a little baby or whether you get it when you're an adult.

Speaker 1:

Why do most men come in to get one later in life?

Speaker 2:

Yeah, you know it's a little bit all over the map. I mean, I get guys coming in for cosmetic reasons. I get guys coming in for religious reasons, whether they're converting to Judaism or converting to Islam. I get guys coming in because they get infections, because they get smegma, which is a type of bacteria trapped under the foreskin. I get guys that have ripping or tearing of the foreskin because it gets scarred and they're not able to pull the head of the penis back over the penis. So really, those are the basic reasons why we do circumcisions.

Speaker 1:

And circumcisions? Do they or do they not affect size, length, etc.

Speaker 2:

No, they don't affect it.

Speaker 1:

I mean, I didn't think so, but I felt like coming from you might be more believable than me. Does hanging weights help with penile enlargement, and is it safe attaching weights to your dick to stretch?

Speaker 2:

You know that's not something I would recommend. Honestly, I don't know very much about it, but it just doesn't seem safe. There are plenty of other ways, including the P-Long protocol, that would be much more controlled and effective than hanging weights on your penis.

Speaker 1:

So you're, I'm sorry. I'm sorry, I don't mean to.

Speaker 2:

As a penis owner, I'm kind of cringing.

Speaker 1:

So what you're saying, doctor, is that people should not tie weights to their dick.

Speaker 2:

Yes, I would definitively say, as a board-certified urologist, yes, I would definitively say, as a board certified urologist, that the American Urologic Association does not sanction tying weights to your penis.

Speaker 1:

You heard it from a doctor, guys.

Speaker 2:

I will speak on behalf of all urologists and probably all men.

Speaker 1:

I appreciate that. I appreciate that. So firm answer on that, don't do that. It's a definite no. I appreciate that. I appreciate that. So firm answer on that, don't do that it's a definite no.

Speaker 2:

Definite no, I did get-, because guys like me end up having to take care of the complications of problems like that.

Speaker 1:

Maybe not your favorite job.

Speaker 2:

Yeah, I mean I remember when I was in residency we had a guy that decided to make love to a weightlifting collar, you know those things. You put on the 45 pound dumbbells and we had to call the, the fire department, in with the jaws of life to cut that, that metal collar on the penis, or you know, I mean I've had guys stick mercury thermometers up their penis or crack pipes up their penis or pencils up golf pencils up their penis or safety pins up their penis, and so you know, like it's, it's all fun and games until you wake me up at two o'clock in the morning and I have to come and take those kind of things out.

Speaker 1:

I guess off the topic, but on the topic, since you brought shoving things up penises. A sounding is a thing. Have you heard of sounding? It's in the king?

Speaker 2:

I mean, if you're going to do it, get a, you know a formal metal, urologic sound and some lubricant. I don't imagine I've placed sounds up guys' penises, you know, hundreds of times and cystoscopes probably up guys' penises probably thousands of times and I don't recall anyone saying that it was particularly pleasurable, but a one in a million. There's still one person out there. Yes, yes, I mean also maybe I just haven't met that person.

Speaker 1:

Maybe if it's a pretty woman doing it. Not that I'm not saying you're not pretty, but I'm just saying some people might.

Speaker 2:

I don't know, like I said, if it's a one in a million, there's still one person out there that's interested in doing it, but I haven't met that person yet.

Speaker 1:

Well, you know, the client who keeps coming back in for that same exact procedure maybe likes it.

Speaker 2:

Yeah, that's true.

Speaker 1:

Just a thought, all right. So, guys, that was a lot of helpful information right there. Now I did have, actually, a couple of veterans who reached out to me and a combination of being put on meds and depression had affected their ability to get erections and also and they had felt like the medication had caused them to not be as long, like had affected their length. Does mood medication actually affect length?

Speaker 2:

Well, I mean there are psychoactive medications like antidepressants and some of the medications for bipolar that certainly can affect libido and can even affect a guy's ability to get an erection, and if you reduce the blood flow to the penis then certainly you'll have decreased penis size.

Speaker 1:

And do you have suggestions for how to counteract that?

Speaker 2:

Yeah, I mean I have a supplement called Affirm, which is a nitric oxide booster, which I just completed a study showing that it increased nighttime erection blood flow by about 25%. And so it's a combination of beets which have nitrates, which is a nitric oxide donor when it's converted into nitrite, and watermelon, which has citrulline, which converts into arginine and is a nitric oxide donor, and so it's a natural supplement that's highly effective in increasing erectile function by opening up blood vessels. In addition, you can just take a daily dose of Tadalafil or Cialis and that also will improve erectile function. And then we have a whole host of technologies in the office, including low-intensity shockwave therapy, platelet-rich plasma, and then medications like brimalanatide or PT-141, oxytocin, apomorphine.

Speaker 2:

I wrote an article that appeared in Maxim called Urologist's Guide to Sexual Superpowers, or you can just go on my personal website, which is BrandeisMD, and there's a link to that article and that kind of goes through. I think 14 or 15 MD and there's a link to that article and that kind of goes through. I think 14 or 15 technologies A lot of them you probably haven't heard of that will help improve erectile function.

Speaker 1:

Well, you brought up, I think, electric stimulation. I do have a question here. One of my experts had brought up Gaines Wave therapy, helping with venous leak, and someone asked me to find an expert who could speak to Gaines Wave a little bit more and why it helps with Venus Leak.

Speaker 2:

Yeah, so Gaines Wave is just kind of a brand, like 1-800 is a way to access dentists, so it's a marketing company that will bring you to a physician who does low-, low intensity shockwave therapy.

Speaker 2:

And there's two forms of low intensity shockwave therapy. One is what's called radial wave, which actually is a pressure wave, not a true shockwave I don't know if you want me to give a physics lesson or not, but and then there's what's called focused shockwave and they're slightly different machines and the focused is probably a little bit better than the radial, but they both work to improve penile blood flow, and the way they do that is they trick your penis or your body into believing that there's an injury, even though there's not an injury. The shockw waves create a inflammatory and a healing response, and part of that inflammatory and healing response is to activate stem cells. Stem cells in your body are cells that can grow into other cells and also cause the release of growth factors, and so when you activate stem cells, cause the release of growth factors, especially vascular growth factors, you'll grow new blood vessels, and so that's one way to regenerate. The ability to get and keep an erection is by boosting blood vessels, which boost blood flow.

Speaker 1:

Okay, all right. Can you explain venous leak really quickly?

Speaker 2:

Yeah, of course. So there's an artery called the deep cavernosal artery that sits on the inside of the corpora cavernosa, which is the erectile body, and you have two erectile bodies, one on the left and one on the right, and as the arteries pump blood into the penis, the corpora or the erectile body swell. Now the return of venous blood sits on the outside of these corpora, and so as the penis swells, the artery right arteries have muscular walls to pump blood. That's why you have a pulse, because arteries are pumping blood but veins have very thin walls. So the return of venous blood is a passive flow, and so it takes a lot of pressure to occlude an artery, but it doesn't take a lot of pressure to occlude or block a vein, and so as the penis is increasing in size, it begins to constrict the return of venous blood through the emissary veins, which live on the outside of the corpora, between two layers of tissue, almost like an ice cream sandwich. And so if you're able to achieve a certain level of pressure in the penis, then you're able to block the backflow of venous blood. So there's a threshold blood pressure that you need to achieve in the penis to prevent the venous black flow.

Speaker 2:

So think of it this way If you're on the roof of a burning building and there's another building that's six feet away, if you jump six feet, it's a great day. If you jump five and a half feet, it's not so good a day. And so, with the penis, say, you have to achieve a pressure of 100 in the penis in order to block the backflow of venous blood. If you get to 100, it's a great night. If you get to 95, it's going to be a frustrating night. You're going to have a full penis, but it's not going to be rigid enough for penetration.

Speaker 1:

And this Gaines Wave, or the therapy that we just talked about, is one of the ways that you can address that Exactly One of the ways that you can help to improve the blood pressure in the penis. One of the ways that you can help to improve the blood pressure in the penis.

Speaker 2:

Someone asked about Peyronie's disease and what your thoughts on that were and how to handle it. Yeah, so Peyronie's disease is scar tissue on the inner lining of the erectile body called the tunica, albigenia right, so tunica is just lining, or tunic, and albigenia is white. So when you actually strip away the skin and the subcutaneous tissue, the lining of the erectile bodies are white and they're chambers and that tissue is very thick. So it's even thicker than the aorta, which is the largest blood vessel in your body. And the reason it has to be that way is because it has to be very strong in order to withstand the pressures of intercourse and has to withstand the intense arterial pressures that are generated inside the tunica and during intercourse.

Speaker 2:

Or about 80% of the time guys don't even realize when it happens, but you get little injuries of that tunica and because of that injury you get an inflammatory response and because of that inflammatory response you get a healing response. And that healing response means you lay down scar tissue. And scar tissue in the body is strong but it doesn't stretch, doesn't have any elastin in it, and so when you get an erection, part of the penis is not able to stretch to the same length that it used to, and so then, all of a sudden, you have a deflection because you have a difference in size between one side and the other.

Speaker 1:

And is there a treatment for it?

Speaker 2:

one side and the other. And is there a treatment for it? Yeah, it's actually a really difficult disease process to treat. So the one FDA approved treatment is a medication called Zyaflex, which is a collagenase. It's an enzyme that dissolves collagen. Remember I said that the scar tissue is made of collagen. So if you inject the medication into the collagen and then you use a penile traction device called the Restorex device, you're able to reduce that curvature by about 30 degrees 35 degrees, sometimes more, sometimes less.

Speaker 2:

The downside of that medication is one it's dreadfully expensive, so it's $5,000 a shot. A typical course of treatment is eight injections, and so insurance companies aren't lining up all excited to pay for that. The other thing is after an injection, when injections are paired. So you get an injection Monday, an injection Wednesday, and then you're not allowed to have penetrative intercourse for four weeks, and then you can have penetrative intercourse for two weeks and then you get another set of injections and you do that for four cycles. And so the downsides are it's expensive, it's painful and it restricts your ability to have intercourse. Another treatment is low-intensity shockwave therapy, and that works in properly selected patients. It doesn't work in everyone, but in properly selected patients. Using focused shockwave therapy and the Restorex device. You can also increase penile length, because that's one of the downsides of Peyronie's disease is a restriction in penile length, because that's one of the downsides of Peyronie's disease is a restriction in penile length and reduce the curvature.

Speaker 1:

So maybe some preventative measures would be not to pound away so hard at her and bend your penis.

Speaker 2:

Actually the best preventative measure is to make sure that when you have penetrative intercourse you have a rigid erection. Diabetics are three times more likely to have Peyronie's disease and I think it's because they have kind of weaker erections and so their erections kind of are more likely to get deflected and their penis is more likely to bend of are more likely to get deflected and their penis is more likely to bend.

Speaker 1:

This. Yeah, and this brings me to a question. I did have someone write in and ask about broken penises, penises being broken, injured to that extent. Is that an actual phenomena?

Speaker 2:

Yeah, it's called a fractured penis and so it usually happens during intercourse, when the woman is on top, they come down on a guy and they miss and the penis goes up against the bone and then gets deflected and the pressures are enough to rupture or crack the corpora, that really tough connective tissue, and that typically results in a pop or a crack sound and blood rushing from the erectile body into the subcutaneous tissue. So you go from a normal looking penis to an eggplant pretty quickly.

Speaker 1:

But it can be treated and fixed.

Speaker 2:

Yeah, sometimes you just recommend rest, but sometimes you actually have to go in and what we call deglove the penis. So you make an incision, like you do a circumcision, pull the skin all the way down, go in and find the rupture and then use what we call a proline or a ethebon like a permanent suture to close up the hole and then put everything back.

Speaker 1:

Ladies, don't come down hard the wrong way.

Speaker 2:

Do not do that, be kind.

Speaker 1:

Oh, one more question before we get to the enlargement options. In my last discussion we talked about how, for some people who are heavy, losing weight makes the penis larger, and I've had a whole bunch of questions about that. I'm going to let you explain what that really means. Losing weight, your dick gets longer. Because I had people write in and they're like how many pounds do I need to lose to gain XYZ inches? But you know that when we discussed it, that's not exactly how he said it. But how does losing weight make you appear to have a longer penis?

Speaker 2:

yeah, I mean, penis size is penis size. It's basically the size of your corpora cavernosa. But you know, you can hide most of the external penis if you have what's called a pannus, right. So so you have a pannus of your abdominal fat, but then you have a pubic pannus or pannus of of fat above the penis, and you know, the bigger that fat pad is, the smaller your penis is going to appear. I mean, I remember when I was in doing general surgery at UCLA, they were starting to pioneer bariatric surgery, where they do you know, they reduce the size of the stomach.

Speaker 2:

And they had this guy. He was a thousand pounds, it was on the news. They had to like cut open the doorframe in order to get him to UCLA, and then we had to use two surgical beds to support the weight of this guy. And then, of course, they called the urologist in to put a catheter, and I just so happened to be the resident on call.

Speaker 2:

So they called me into the operating room and so I had the scrub nurses use a broomstick to hold back his abdominal pannus and then use a broomstick to hold back his abdominal pannus, and then the two surgical residents grabbed the pannus over the penis and then I used they're called a nasal speculum it's what the ear, nose and throat docs use to get way down deep into the nasal area to open up the guy's foreskin, into the nasal area to open up the guy's foreskin and then a headlight and long pickups to place the catheter in the guy's penis. So you know his penis wasn't positive length, his penis was about negative six or eight inches. So you can have a panus that subtracts length from the penis to go to a point where you actually have negative length.

Speaker 1:

So if you want to become a urologist, just know you may find yourself with a headlamp.

Speaker 2:

I've been in some deep, dark, strange places. Let me tell you.

Speaker 1:

How did he pee? Do you know how did he pee?

Speaker 2:

Hey, do you know how did he pee? I mean it just kind of dribbled out and overflowed the foreskin.

Speaker 1:

Right. So the point of this is the more in shape you are, the more of your penis is going to show on the external side of your body.

Speaker 2:

If you trim your pubic hair, it's going to appear longer also.

Speaker 1:

Yeah, I mean, guys do that all the time, all the time Like really manscape, which is nice, I like it, I like a solid manscape, but we also know what you're doing when you do it. Just so y'all know. We're like oh, now we're going to talk about guy certified expert, about guy certified expert certified ways to enlarge your penis that are safe. First of all, are there any natural?

Speaker 2:

ways. Pee-long is the only natural way to increase the length, girth and function of a healthy man's penis, right? So this? We're not talking about guys with erectile dysfunction because they don't have enough blood flow to fill up the penis that they have. I'm talking about guys between the ages of 20 and 50, 55, 60 that have normal erectile function, that want to specifically increase the length, girth and function of their penis. And I'm not advocating that people do that. Right, that's your own personal choice.

Speaker 2:

But as a urologist and a sexual medicine expert and a surgeon, I've seen a lot of men come into my office with problems from trying to increase the length, growth or function of their penis.

Speaker 2:

So people use hyaluronic acid or PMMA, which are fillers, right, but those fillers can get infected. Those fillers desiccate, meaning the water content of those fillers goes away, and so when you first inject those fillers into the penis, it actually looks like the penis is much bigger. But within a year or two most of those fillers have dissipated and then you end up with a lumpy, bumpy penis. And so you pay $10,000 up front to get a fat penis and then two years later you end up with a smaller, lumpy, bumpy penis and then you have to pay another 2,500 bucks every couple of years to get more filler injected in the penis. And the other thing is it's not a long penis, right, it's just a thick penis, and so I call that a pig in a blanket penis. And so pig in a blanket is great cocktail parties, but I don't think it's really great in bed.

Speaker 1:

Man-certified.

Speaker 2:

Man-certified right. So you can also do what's called a fat transfer. So you can take abdominal fat, spin it and then inject that below the skin and above the corpora, but you get a squishy penis. The fat doesn't have the firmness that a normal erection has, so I definitely wouldn't recommend that. And then a while back they were doing what's called suspensory ligament ligations. So they would go and cut the ligament that attaches the penis to the underside of the pubic bone and that would give better flaccid length. So the penis would hang lower. But the problem is that generated scar tissue and that scar tissue eventually would restrict the interior penis from getting bigger and pushing the exterior penis out, and so in the end guys would end up with smaller penises and because it was hanging lower, when they got an erection it would point towards their toes instead of towards their nose, and so I would definitely recommend that.

Speaker 2:

And then there's something called the penuma or the implant, which is a silicone taco that is placed under the skin and above the erectile body and that increases flaccid length.

Speaker 2:

It also increases girth significantly, both in the flaccid and the rigid state.

Speaker 2:

The problem is one it's about 20 grand to get one of those things, and the other thing is that I've seen, and many, many of my colleagues have seen, disasters from this where guys get infections, they get bleeding, they need to have them removed and then patients need multiple surgeries to have things restored back to normal.

Speaker 2:

And I know that there's a number of class action lawsuits against the inventor, including one that one of my patients was part of. So I wouldn't recommend that, and that's why I developed the P-Long protocol, not because I really care about how big a guy's penis is, but I didn't want men to hurt themselves and to take a perfectly normal penis and cause problems for it. And so, as someone who has a background in clinical research, I did research at American Red Cross, I did research at Harvard Medical School, I did research at UCLA, I created a protocol, got it institutional review board approved, had it listed by clinical trialsgov, which is the NIH website, and then I presented the outcomes at the Sexual Medicine Society of North America, at the International Society of Sexual Medicine, and then had the data published in Andrology, which is a reviewed medical journal, because I wanted to show like this actually works.

Speaker 1:

Right, right. So can you walk us through what it looks like, how it happens?

Speaker 2:

Right. So to increase the length of the penis we recommend using the Restorex device, which is a device developed by a friend of mine at the Mayo Clinic that's been shown in Peyronie's disease patients to increase the length of the penis about 1.7 centimeters. It has a unique way of grabbing the head of the penis to stretch in length. But you also want to increase in girth. You don't want a pencil penis, and so there's a penis pump by the Dr Joel Kaplan people that has a gauge on it that helps us determine how much pressure to use to increase the girth of the penis. So with the Restorex device, you use it 20 minutes in the morning, 20 minutes in the evening. With the penis pump, you use it 12 minutes in the morning, 12 minutes in the evening. And then we have patients take the Affirm nitric oxide boosting supplement to increase the vascular blood flow and help improve nighttime erections.

Speaker 2:

And then the secret sauce is PRPR, platelet-rich plasma. So platelets in the body have two functions. One function is to cause clot, to stop bleeding, but the other function is to start the regrowth process and it does this by having a lot of growth factors inside platelets. So when you get injured, platelets come, they cause a clot, they stop bleeding, you get a scab. The scab falls off and that tissue grows back. And the reason that tissue grows back is because the platelets release growth factors and that growth factors start the regrowth process. And so patients in the P-LONG study got an injection of PRP once a month for six months and, believe it or not, it really is not very uncomfortable to do those injections. And what we found was that guys got 0.8 inches of length and 0.45 inches of girth added to their penis and an improvement in erectile function because of the PRP.

Speaker 1:

So you're doing this all together at the same time, all together. And how long does this go on for and does it stay?

Speaker 2:

So the P-LONG study lasted six months, right? So when you do a study you have to decide that there's going to be an endpoint where you can statistically significantly show that you have created a change, and so that you know. For me that was six months and in six months guys' penises grew about 15% in length and 10% in girth. The question is okay, if you do it more, are you going to get more length and girth? And I have a lot of patients from the P-Long study and other patients on the P-Long protocol that are going beyond six months that continue to grow in length and girth. And as long as you're getting good, adequate nighttime erections a normal guy should get 30 to 60 minutes of erections every night as long as you're getting those erections, then you're stretching your penis. And as long as you're stretching your penis, then you should be able to keep the gains that you get from the P-Long study.

Speaker 1:

One of the questions I had forgotten to ask you was and I guess, since it's integrated into your protocol here was the safety and effectiveness of penis pumps.

Speaker 2:

I mean, when used properly, it's a totally safe device. You know if you're buying one on Amazon for 50 bucks, you're probably not getting a safe device. You know there are medical grade pumps like the ones from Dr Joel Kaplan that have been around for 20, 30 years and you know, within the PILONG protocol, are effective for increasing girth. I use them a lot in men who are over say, 60, 65, 70 years old that aren't getting nighttime erections anymore, because nighttime erections are really important for penile health, and so I recommend guys in those situations pump. In addition, you can get an erection adequate for penetration by pumping, where you pull venous blood into the penis and then stick a ring around the base of the penis to keep that blood trapped in the penis.

Speaker 1:

A good cock ring.

Speaker 2:

You know, what we see sometimes in the P-Long study is guys that are a little over enthusiastic and so if you generate pressures that are too high, you can begin to rupture little capillaries and so you'll get some purple dots on the penis or purple head of the penis or even some blisters on the penis or purple head of the penis or even some blisters on the penis. So if you're getting those kinds of things, you're pumping at a pressure that's too high. I mean, it's like you know, guys right, if 10 is good, 11 is better and 12 is the best. But you know, sometimes too much is not great, not a good thing.

Speaker 1:

And just to be clear, this, the peeling process it does. It make sex feel better. It doesn't, it definitely doesn't hurt sexual fulfillment.

Speaker 2:

Well, you know, guys have a longer, girthier penis that works better. Well, if that translates into feeling better, great.

Speaker 1:

But they probably won't notice the difference in how they experience sex, just how it looks and how it feels.

Speaker 2:

Yeah, but I mean, if a guy has more confidence because he has a bigger penis, and that's what his goal is then yeah, then he's going to experience it as better.

Speaker 1:

Confidence is definitely super important when it comes to sexual.

Speaker 2:

And that's the interesting thing to me about all these other interventions the fillers, the implants and those kinds of things is that I mean, I'm not a woman. But if I were a woman and I saw a man who had taken one of those interventions to increase the size of the penis, that might be a turnoff, because here's a guy that's not really confident in who he is and what he has and whereas the P-Long it's a totally natural penis. Right, there's no fillers, there's nothing artificial. You just have a longer, thicker penis. That works better.

Speaker 1:

Confidence is a huge turn on, I think, for most people, hopefully of all gender. But I do think that if your concern with the size of your penis is affecting your ability to show up for your partner or to go after getting a partner, your ability to show up for your partner, or to go after getting a partner, then that's a legitimate reason to seek help.

Speaker 2:

Come to p-longcom and we have all the information that you need to learn more about the P-LONG protocol, and you know. The other nice thing is we have data on what you can expect, right. So if you go in there with a four-inch penis and think you're going to end up with a 10-inch penis, you're barking up the wrong tree. I'll give you very clear expectations of what you're getting for the money and the effort that you're going to put into it, Because I think that's really important to be very transparent and honest with patients.

Speaker 1:

Absolutely, and just to be clear, transparent and honest with patients. Absolutely, and just to be clear. I don't know a lot of women that want a 10-inch dick, like I mean. I know there are some. I know there are size queens out there, but there are plenty of women that aren't going to want you to try and go from 4 to 10. I believe that 100%, so do you have any final thoughts? You want to leave my listeners with.

Speaker 2:

You know I mean. What I would say is the function of the penis is to urinate and for procreation, and 98% of men have penises that are adequate for procreation and for urination. So increasing the size of the penis is a choice. Now, the reason I created the P-Long protocol is to give you a very safe and healthy way to naturally increase the length, girth and function of the penis. So if it's not something that you're driven to do for whatever reason, you know I want you to have something that's safe and effective. Don't do fillers, don't do fat transfers, don't do ligament ligations, don't do a silicone implant. You know, if you're interested, just go to p-longcom. You know it's a six-month protocol to get you about 15% increase in length, 10% increase in girth and better function, and if you want more than 15%, then you can continue, and so that's what I would say.

Speaker 1:
